CLINICAL NURSE SPECIALIST - LEWES - E SUSSEX
Full or Part-time - up to £37,665 FTE

Our client is based in Mid Sussex and they care for patients from Burgess Hill, Haywards Heath, Lewes, Uckfield and the surrounding villages. They provide support to the community to live well towards the end of life by giving compassion, hope and quality care. They want to make a positive difference to the experience of everybody who faces death or bereavement, by offering choice and support through expert care, knowledge and understanding.

A vacancy currently exists for an experienced Clinical Nurse Specialist. Their team of six Clinical Nurse Specialists provide advice, support and liaison for people in East and West Sussex helping to plan their care and support and ensure the provision of expertise around specialist symptom control and emotional and psychological support for patients and those important to them. They are supported by a Lead Consultant.

You should possess excellent communication skills and the ability to work as part of a multi-disciplinary team as well as on your own initiative. You will be a key link between the Hospice and our primary and acute care providers. MAIN DUTIES AND RESPONSIBILITIES

1 Managerial

To work as a member of an inter-disciplinary team to develop St Peter and St James' philosophy of care.
To follow hospice procedures and policies and the Code of Professional Conduct as laid down by the NMC.
To promote and teach the principles and practice of palliative care in the home to the patient, family and primary health care team.
To support, advise and liaise with the inter-disciplinary hospice team, primary health care and hospital inter-disciplinary teams.
To promote and participate in teaching and training programmes for Hospice, community and hospital based staff.
To ensure that the patient's physical, psychological and spiritual needs are met.
To ensure that the psycho-social needs of relatives are met; identifying relatives requiring long term support and discussing them with the St Peter and St James Counsellor regarding appropriate follow up.
To maintain and update records, and any other statistics as necessary.

2 Clinical

Together with the inter-disciplinary hospice, hospital and primary health care teams:

·To hold a personal workload of Hospice patients in the community.
Assessing the specialist needs of patients and their families in the community prior to hospice discharge and also, where appropriate, those patients attending day care.
Planning and offering specialist advice on the care which will meet these needs and liaising closely with senior members of the medical team.
Evaluating the advice given.
Ensuring that patients and relatives are actively involved with the planning and, where appropriate, the administration of care under supervision.
Ensuring that all relevant information is recorded in the hospice inter-disciplinary notes, together with any written reports to hospital and primary health care teams.
Being available to staff/patients/relatives for psycho-social support.
Ensuring that the patient's physical, psychological and spiritual needs are met.
Ensuring that the appropriate community patients are referred for day care.
Ensuring that patients are referred for comforting therapies as appropriate.
Ensuring that patients are referred to the physiotherapist as appropriate.
Ensuring that patients are referred to occupational therapist as appropriate.
Ensuring that patients are referred to counsellor as appropriate.

3 Teaching and Training

Together with senior nursing staff:

Acting as a role model clinically for both trained and untrained nursing staff.
Initiating, facilitating and participating in the formal teaching of both trained and untrained nursing staff through planned education programmes in conjunction with other members of the St Peter and St James team.
Assisting in the orientation programme for new members of staff, nursing volunteers and visitors. Acting as mentor is required.
Participating in the ongoing educational programme of the hospice by attending in-house study days and courses as appropriate. Taking responsibility for attending mandatory training, such as fire and lifting and handling, in line with current legislation.
Keeping abreast of current advances in nursing practice and research and to take some responsibility for his/her own professional updating (PREP); taking responsibility for maintaining clinical competencies.
Participating in reflective practice/clinical supervision and individual performance review (IPR).

Quality Assurance and Research

·To assist the Head of Nursing & Clinical Care and other members of the inter-disciplinary team in initiating and promoting research projects.

·To be aware of new developments in palliative care.

Together with the Head of Nursing & Clinical Care, to develop
standards of Care and Audit Protocols.

·To attend Clinical Governance if other members of team are unable to attend.
