Band 7 - £33,222 - £43,041 pa pro rata

Band 6 - £28,050 - £36,644 pa pro rata

Band 5 - £23,023 - £29,608 pa pro rata

Northamptonshire Healthcare Foundation NHS Trust’s Intermediate Care Team has been awarded greater investment and is expanding the service.

We deliver an integrated patient-led, high quality health and social care community service for all adult residents throughout the county of Northamptonshire within their own home. We are the NHS provider of choice.

We have a number of clinical pathways which support our patients within their own homes; this includes treating and supporting patients with medical needs to remain at home by preventing hospital admission through Rapidly Responding. As well as rapidly discharging people who reach A&E in both our acute hospitals in the county by in reaching into acute care through our nursing and emergency care practitioner team.

We also facilitate rapid discharge from the main ward areas of our two acute trusts and community hospitals through the discharge to assess model which is led by our dynamic therapy team. The therapist also lead on a non-weight bearing pathway to prevent long stays in acute care.

We also provide all Community Intravenous Therapy in the county.

  • As a junior member of staff are interested in a rotational post to gain a greater knowledge base and diverse skill set.
  • Feel passionate that where possible patients should remain in their own home or return home quick to prevent pyjama paralysis.

If you feel you have the necessary skills and competencies to fulfil our roles, are excited about the opportunities that Unplanned Care/ICT offers and are willing to take up the challenges of working across professional boundaries, we are the team for you.

We are looking to attract various clinicians with an interest or relevant knowledge and skills and an ability to work autonomously seeing a wide range of clinical conditions with patients in their own home. Our hours of work are from 7am to 1am across 7 days a week, with a range of shift for our nursing staff and support staff and between 8.30am to 6pm across 7 days a week for our therapy staff.

For Nurses we are able to offer between our service times of 7am to 01am a range of shifts over 6hrs, or 7.5hrs & or 12hrs or 12.5hrs.

We are able to offer full and part time hours and 0 contract hours on our bank.
