Hospital Liaison Nurse (Learning Disabilities)

Salary: £46,048 - £49,040  per year

There is an exciting opportunity for a suitably qualified learning disability nurse (RNLD/RNID) to lead and develop services for people with a learning disability working within Jersey General Hospital.

The Learning Disability Liaison Nurse role exists to ensure that the needs of the person with a Learning Disability are met when they require admission to Hospital. This involves communicating with, and working in partnership, with a range of clinicians and services working to develop systems and educating staff where appropriate. It also involves close communication and involvement with family, carers and others during the admission period. The successful candidate will provide leadership within the Hospital to ensure that services are provided in line with local policy & guidance for people with Learning Disabilities, which promotes their rights and inclusion as an autonomous practitioner to assist the Hospital ensure a smooth transition between pre admission, admission and discharge arrangements.

The post holder will act as a point of contact for family, carers and other professionals during the service users journey and assist in managing the interface between mainstream Hospital, primary and specialist learning disability services and develop effective and collaborative working with healthcare and multi-agency colleagues, including identifying and meeting bespoke training needs and nursing advice.
