An excellent opportunity has arisen for a Nurse Care Manager to join Ark Health and cover Nottinghamshire through to Doncaster, including Bassetlaw and Gedling. In return, we offer a competitive salary of £38-40k per annum.

The Nurse Care Manager will report to the Head of Clinical and will be responsible for the delivery of high quality care to patients within their own home, or another community setting. The role will involve working closely with the complex care team and promote health/wellbeing and independence for patients on the team caseload, working closely with other Partners/stakeholders including GP’s, Physiotherapists, Occupational Therapists, District Nurses, keeping clients in their homes, also overseeing facilitation of hospital discharges.

The Nurse Care Managers main duties and responsibilities will include:

  • Work in accordance with NMC Code of Professional Conduct and scope of Professional Practice and in accordance with the company’s aims, objectives, ethos and values
  • Assessment, planning, implementation and evaluation of allocated customer needs
  • Assess new customers’ needs, following referral and provide detailed feedback on the clinical and social aspects of the care / support required
  • Ensure all new staff are assessed for competence and that they are clinically safe to practice including devising a training and competence action plan to assess, train and sign care workers off as competent
  • Ensure existing staff are monitored to ensure competency levels are up help and maintained by completing quarterly spot checks on staff within your caseload allocation
  • Ensure that Staff are performing their duties in a controlled manner that is safe, effective and caring and that promotes dignity and privacy
  • To complete monthly customer reviews
  • Identify risks through the assessment process documenting and initiating appropriate action, reporting as appropriate to Specialist Nurse
  • To undertake clinical training to new care workers once CIS induction has been completed
  • To set up new packages of care with the support of the operational team
  • To ensure KPIs are achieved and maintained for auditing and quality purposes.
  • To develop and maintain evidenced based practice
  • To contribute to the protection of individuals from abuse by reporting any incidents involving potential or actual abuse and acting upon concerns immediately and informing the senior clinician on duty
  • Recognise deterioration in customer’s condition advising Head of Nursing or delegated senior practitioner of any circumstances which give cause for concern

    The successful Nurse Care Manager will have:

  • RGN with valid NMC PIN
  • Minimum 2 years’ experience in Nurse Care Management
  • Strong management and prioritization skills
  • Excellent communication skills, both written and verbal
  • Community based experience would be advantageous
  • Skilled in advanced respiratory ventilation ideally
  • Completion of the ITU course would be advantageous

    Applicants must be prepared to travel across the geographical area and occasional overnight stays
