£22,128- £35,577 - 37.5 hrs per week - Mon- Fri 8.30 to 4.30

Vacancies have arisen for qualified nurses who want to be part of a team to deliver this innovative model of care in the Ashford, Canterbury and Margate areas to create measurable improvements in population health. As an individual, you are results-orientated and enjoy engaging with, motivating and enabling patients to prevent avoidable future health care needs. You are a good listener and are able to quickly understand a patient's situation in a holistic manner. You enjoy problem-solving in collaboration with patients and the wider multi-disciplinary team and are structured in your approach to addressing the patient's needs.

Proactive Health Coaching is a mainly telephone-based health- service that helps patients understand their health conditions, plan their care, and find their way around the healthcare system.

The post-holder will:

1.Meet patients and jointly develop an action plan for how their health and quality of life can be improved and future care needs prevented . This is done on the telephone for some months after an initial face-to-face meeting.

2.Coach and motivate patients to follow their individualized plan and care pathway, supporting them to make proactive choices.

3.Engage and effectively communicate with other health, social care and voluntary sector providers to holistically support the patients' health and social care well-being.

4.Share and learn experiences with working with patients during weekly discussions with colleagues, focusing on specifically challenging patient cases.

5.Participate in regular meetings focusing on continuously improving the quality and be part of implementing improvements.

6.Positively engage in a defined research methodology, working within the approved ethical guidelines.

3.0 Key Responsibilities:

3.1 Clinical
1.Identify risk factors, and work with patients to develop a goal orientated plan to minimise the risk of avoidable admission.
2.Contact patients on the telephone prioritizing by need and risk.
3.Provide ongoing motivational dialogue.
4.Contact care providers to help support the minimisation of risk.
5.Work within the restraint of ethical approval and guidelines.
6.Provide feedback to the local health care providers.
7.Signpost of other supportive agencies, e.g. AGE UK.
8.Develop a 'local knowledge' of the locality and its services and facilities, particularly in relation to health and social care services.

3.2 Supervision and Training
1.Undertake appropriate one to one meetings on a regular basis in accordance with the relevant professional guidelines and policies.
2.Progress through the Health coach training program, attaining the relevant certification and qualification.
3.Support and facilitate the training of newly employed health coaches.
4.Attend locally organised and national training appropriate to the work role, authorised by your line manager.
