We are looking to recruit a dynamic and enthusiastic qualified Learning Disability/Intellectual Disability Nurse to join the team within Adult Services to provide healthcare and support to people with a learning disability to help them to achieve a fulfilling life. The post holder would be expected to hold a caseload within a Care Coordination role and work within a multi-disciplinary approach. This senior role requires the candidate to have a minimum of 4 years registration, demonstrate good clinical assessment skills and can demonstrate effective planning and prioritisation of care delivery.

The successful candidate will be working within a community hub with other members of the multi-disciplinary team members providing therapeutic interventions in order to achieve successful outcomes for the people they work with.
