Thames Hospice is delighted to be able to announce two new senior roles within our Community Team. We are looking for two experienced community nurses to join us as Team Leaders. You will work directly for the Head of Community Services and with the Single Point of Access Team Leader. The Team Leaders will work in the proactive case management part of the team but will maintain very close links with our 24/7 Advice Line and Rapid Response Teams.

You must have experience of community nursing and palliative care. The roles will maintain a clinical component and you will hold your own caseload. The role will also include managing a team of Clinical Nurse Specialists, Staff Nurses and Health Care Assistants.
