Nurse Practitioner with Chronic Disease Management and Minor Illness Skills
St Helena Island, South Atlantic
2 year FTC commencing September 2018
Salary £38k pa, taxable in St Helena

The ideal candidate will have a varied role which combines staff and service development with seeing people with acute urgent conditions and supporting the effective management of those with long term chronic diseases. The main element of the role would be to support the design and implementation of contemporary and evidence-led practices in holistic management of chronic diseases that are prevalent on the island. These are mainly diabetes, hypertension, chronic kidney and respiratory diseases.
You will work with a team of committed and dedicated local and expatriate nurses seeking to promote healthier lifestyles, reduce complications from established long term conditions and provide evidence-led clinical care on the island.
The post is full time. Demonstrable competence, skills and previous experience in chronic disease management, providing care for long-term condition and treating minor illnesses will be essential. Successful applicants will have significant primary care and community nursing background, able to manage teams and services and will be an effective team player. You will be passionate about patient care and outcomes, demonstrate critical thinking in the clinical decision making process, be able to work with and treat patients autonomously based on local protocols, be flexible and forward thinking to embrace new ways of working and transfer skills to local nurses to facilitate extending the role of the nurse practitioner to the wider nursing team.
