Band 7 - £31,696 - £41,787 pa pro rata

Band 6 - £26,565 - £35,577 pa pro rata

Band 5 - £22,128 - £28,746 pa pro rata

Northamptonshire’s Intermediate Care Team is proud to deliver an integrated patient-led, high quality health and social care community led service for residents in the Northamptonshire County as the NHS provider of choice.

We have a number of clinical pathways which support our patients within their own homes; this includes treating and supporting patients with medical needs to remain at home by preventing hospital admission through Rapidly Responding and also, rapidly discharging people who reach A&E in both our acute hospitals by in reaching into their front doors which is led by our nursing team. We also facilitate rapid discharge from the main ward areas of our two acute trusts through the discharge to assess model which is led by our therapy team. And we also provide a single point of access for All Community Intravenous Therapy in the county.

We are an established multi-professional clinically led community team, and our reputation is well regarded through our ethos of integrated partnership working, by achieving our aims through our proven track record in the health economy in Northamptonshire.

This is a unique role to positively influence patient care if you have:

  • Clinical expertise in working in rapid assessment/admission avoidance/facilitated early discharge for patients with complex needs.
  • Effective leadership skills at all levels
  • Ability to effectively balance a primarily clinical role with managerial responsibilities (band 7 only)

We are looking to attract various clinicians with an interest or relevant knowledge and skills and an ability to work autonomously seeing a wide range of clinical conditions with patients in their own home. Our hours of work are from 7am to 1am across 7 days a week, with a range of shift for our nursing staff and 8.30am to 6pm across 7 days a week for our therapy staff.

We are able to offer full and part time hours and 0 contract hours on our bank.

£3000 RECRUITMENT INCENTIVE

An attraction payment of £3000 for paypoints stated above will be paid to external candidates appointed to this post in nursing and physiotherapy (full details and terms that apply can be found in the NHFT Nursing Attraction Strategy document, attached, and on the Trust website).

Qualified Nursing - ICT Incentive applies as below:-

Band 7 - spinal points 26 to 29

Band 6 - spinal points 21 to 23

Band 5 - spinal points 16 to 18

Physiotherapists - ICT Incentive applies as below:-

Band 7 - spinal points 26 to 19

Band 6 - spinal points 21 to 23
