There is an opportunity for the role of Single Point of Access Team Leader.

We are looking for an experienced community nurse to join us as one of our Team Leaders. You will work directly for the Head of Community Services and with the Community Specialist Nurse Team Leaders. The Team Leader will work in the 24/7 Advice Line and Rapid Response Team, but maintain very close links with our proactive case management part of the team.

You must have experience of community nursing and palliative care whilst maintaining a clinical component to the role. The role will also include managing a team of Clinical Nurse Specialists, Staff nurses and Health Care Assistants.
