Job Summary

Care UK is building on its extensive experience in Primary Care. We are currently designing and making plans to deliver a new model of care for general practice; Practice Plus. Working with a leading Pharmacy Partner, the model aims to provide an enhanced general practice service to NHS patients through the use of technology, remote care and local clinical services delivered from our Pharmacy Partner sites. This new way of delivering general practice services in the UK will utilise Care UK’s extensive clinical contact centre capability as well as optimising patient experience and service quality through use of a comprehensive online capability. As we launch our new model of care we are seeking to recruit a Lead Nurse to join our team in Brighton working across our two practices. The Lead Nurse will establish inter-agency working to ensure good communication increases the quality of care for patients. Their own clinical practice should be consistent with the autonomous role expected of an advanced nurse practitioner The leadership role will encompass motivating and inspiring a team of nurse practitioners, practice nurses, health care assistants and pharmacy technicians and a sexual health team. The lead will also play an integral role within the management team comprised of the service manager and lead GP. The centre offers a range of comprehensive services to registered patients and a walk-in service for the treatment of minor injuries and ailments, without having to make an appointment. The centre is open every day from 8am to 8pm, including all bank holidays and is located just outside Brighton Station, with our second new site a GP practice service for registered patients. Shift pattern would be variable to cover: 8am to 8pm

The Requirements

In order for you to be considered for the job, as a minimum we are looking for you to: • Be a qualified RGN with valid NMC PIN number • Have previously worked as an Advanced Nurse Practitioner within a GP practice/ Walk-in Centre setting. • Hold an non-medical prescribing qualification (V300) • Have previous experience at a senior level within an Acute or Primary Care setting. • Have experience of performance management and supervision of clinical and junior staff • Have experience of investigating clinical incidents and monitoring clinical KPIs. Your Key responsibilities Leadership of a team, working collaboratively with senior management Assess patients attending with a range of signs, symptoms and with undifferentiated diagnoses. Make a diagnosis using a range of validated clinical guidelines and act to provide the best health intervention for the patient Set standards for care by implementing protocols and establishing health education activities. Develop new and innovative tools to assist patients and those supporting their care to do so to their best effect. Maintain accurate documentation of assessment, planning and treatment of patients.

This is a permanent full time job which is 37.5 hours per week, Mon - Fri with occasional weekend shifts.
