Greater Manchester Crisis Care Pathway

New services.  New opportunities.  An exciting future.

Across Greater Manchester at least £13 million is being invested in transforming mental health care for children and young people. 

The Greater Manchester Crisis Care Pathway will see lots of new and improved services.

We need exceptional and passionate staff on board to make sure young people receive the very best care and can reach their full potential. 

As part of the pathway, four new rapid response teams are being created.  These will provide assessment and case management to young people who are experiencing a mental health crisis.  The teams will also offer intensive post-crisis support to children, young people and families.

If successful, you will mainly work in one area of Greater Manchester – and you can specify your preference (see below).  Sometimes there will also be a need to work across the whole of Greater Manchester, which will offer you a great opportunity to further develop your skills and experience.

Four trusts provide the rapid response teams. The posts are being advertised by these four trusts as follows:

  • Pennine Care NHS Foundation Trust (the north team covering Bury, Rochdale and Oldham and the south team covering Tameside and Glossop, Stockport and Trafford)
  • Manchester University NHS Foundation Trust (the central team covering Manchester and Salford)
  • Greater Manchester Mental Health NHS Foundation Trust in partnership with North West Boroughs Healthcare NHS Foundation Trust (the west team covering Bolton and Wigan).

Role summary

You will have the opportunity to make huge improvements in how crisis care is experienced by children, young people and those who care for them. As a member of the rapid response team, you will be at the heart of a well-funded, well-organised and hugely valued service.

You will link with emergency services and primary care colleagues to offer first response assessment and support for children and young people experiencing an actual or emerging crisis.

You will offer intensive follow-up support for up to 72 hours and develop longer-term care plans to identify and address the all-round needs of young people and families.

You will also link with specialist in-patient services, referring and co-managing admissions within a recovery model.

At the very heart of the rapid response teams (and the overall crisis care pathway) is a commitment to providing accessible, high quality and recovery-focused care.  This care will be tailored to meet the all-round needs of every young person.

Key responsibilities of the post

  • Deliver community-based assessment, de-escalation and interventions to young people presenting in crisis, within a new assessment and reduction of risk framework
  • Offer up to 72 hour intensive support and intervention, jointly with colleagues from voluntary, charity and social enterprise organisations and other agencies across Greater Manchester
  • Work across traditional boundaries to develop holistic, recovery focused care plans to promote resilience and prevent relapse
  • Refer to specialist in-patient services and co-manage admissions to reduce length of stay
  • Provide (or provide access to) a range of quality psychotherapeutic interventions that meet the needs of each child or young person
  • Provide advice, guidance or consultation to team members and other professionals, contributing directly to children and young people’s formulation, diagnosis and treatment plan

 

Key requirements of the person specification

You will need to have:

  • Relevant core professional qualification in mental health or social care, plus evidence of continued professional development in a child and adolescent mental health service-related area
  • Evidence of professional registration
  • Substantial experience of risk assessment and risk management
  • Experience of assessment, formulation and managing complex cases and risk within a mental health service
  • An extensive range of clinical experience within the speciality, or within a care pathway
  • Experience of exercising clinical responsibility for patients’ care, treatment and case management
  • Experience of working in partnership with service users and their families and carers
  • A genuine and passionate belief in the child-centred, needs-led, recovery-focused values base of the service
  • Recognition of the value of flexibility and availability for young people. As such, you will need to be able to work weekends, unsocial hours and have use of a car or suitable means of transport
