In recognition of our success Northamptonshire’s Intermediate Care Team has secured funding to expand the team. We are proud to deliver an integrated patient-led, high quality health and social care community led service for residents countywide in Northamptonshire as an NHS provider.

Our Rapid Response Team supports patients with medical needs to remain at home by preventing hospital admission and access support services according to personal need. We are an established independently led community team and our reputation is well regarded through our ethos of integrated partnership working in the health economy in Northamptonshire.

We have a number of clinical pathways which support our role, this includes being the single point of access for All Community Intravenous Therapy. We also facilitate rapid discharge from both the front and back door of out two acute providers ensures a swift discharge back into the community.

Our patients are our priority and they regularly feedback their positive experience of our service. We strive to support them at their most vulnerable so that they receive quality seamless care and maximising their independence.

This is an exciting opportunity for an experienced and enthusiastic Nurse to join the diverse and challenging team working in Unplanned Care across Northamptonshire.

Are you:

  • A motivated, nurse with experience in a wide range of service areas and conditions?
  • Would you like experience working within a community setting?
  • Are you able to work autonomously and as part of a large dynamic and evolving team?
  • Do you have the necessary skills to supervise other members of the MDT and willing to mentor Students?
  • Keen to progress your career into a more Senior Role?

We are looking to attract Band 5 Community Staff Nurses with an interest or relevant knowledge and skills and an ability to work autonomously seeing a wide range of clinical conditions with patients in their own home. Our hours of work are from 7am to 1am across 7 days a week, with a range of shift.

We are able to offer full and part time hours and 0 contract hours on our bank.
