An opportunity has arisen for an experienced Registered Professional to join the Continuing Healthcare Team. This is a county-wide service and we’re looking for, a highly motivated practitioner with excellent assessment and communication skills.

This is a rewarding and interesting role which will enable a nurse, therapist or social worker to work in partnership with multidisciplinary teams across Health and Social Care establishing healthcare needs and supporting high quality care for clients and their families in the community.

This job has two key elements; To complete a comprehensive assessment to determine eligibility for NHS Continuing healthcare; To provide a case management service for eligible clients for an appropriate package of care or placement.

You’ll be allocated a mentor and will be fully supported through your induction. You will need to be a car driver as you’ll need to carry out assessments across Buckinghamshire.

There’s an opportunity to work flexibly to meet the needs of the service and this includes some working from home.
