OB SUMMARY

The post holder will be responsible for assessing callers who contact the Leicester, Leicestershire and Rutland Clinical Navigation Hub, who present with a wide spectrum of clinical conditions, providing self-care advice and where appropriate advising patients on next steps and referring them on to further care when appropriate.

The clinical triage nurse / paramedic will be required to work as part of a multi-disciplinary team within the service provided by DHU providing quality; evidence based health care to meet the immediate needs of the patient.

Demonstrating a commitment to a high quality service, excellence in clinical practice and continuous professional development are integral to the role and to the development of the service.

**It is expected that this position will cover 24/7 shifts working according to the needs of the service inclusive of bank holidays and 5 out of 8 weekends. This job description is not exhaustive, and it is expected that the post holder will be flexible in their approach, and undertake any reasonable duties as requested by Management/Directors.**

Key Responsibilities

This role will have numerous responsibilities across Clinical services including;

Receive referrals from NHS 111 and healthcare professionals.
Undertake a comprehensive telephone based clinical assessment without reference to clinical decision support systems.
Use clinical knowledge and advanced communication skills to provide high quality advice on self-care as appropriate though the effective communication of evidence-based health care advice.
Facilitate the patient pathway through primary, secondary, community and tertiary care. Where possible assisting patients in the appropriate use of primary care and emergency care services and other healthcare related organisations.
Use negotiation skills as necessary and establish concordance with patients on appropriate course of action to take for the level of care determined.
Maintain accurate patient care records as required.
Manage all calls in a controlled and professional manner treating all patients with compassion, empathy and respect.
Support the achievement of quality standards and performance levels relevant to the service.
Work within the parameters of own skills and knowledge maintaining an awareness of service and practice developments.

The successful applicant will also have duties including:

Participate in the clinical governance and quality improvement requirements of the DHU.
Participate in the reflective and clinical supervision framework for clinicians within the service.
Offer appropriate advice to other clinicians on care practices, delivery and service development within the parameters of the role and scope of practice.
Maintain an active registration with your regulatory body – either the Nursing & Midwifery Council (NMC) or Health Care Professional Council (HCPC).
Work within the professional Code of Conduct and other directives as defined by the relevant regulatory body (NMC or HCPC)

The person we appoint to this role will likely be able to demonstrate the following:

Sub-part 1 level registration with the NMC or Registered Paramedic
Recognised Minor Illness, Injuries certificate
Prescribing qualification (Desirable)
Significant post registration training/consolidation & experience in at least two healthcare settings including one in an aspect of adult general care
Experience of working within a multi-professional / multi agency environment
Broad clinical knowledge base
Evidence of continuous training, learning and development
Previous experience of telephone triage / telemedicine. Aware of current structures and developments in health and social care
Understanding of patient management in a primary, secondary, community and tertiary care Setting
