Job Description

Based at The Wellington Hospital, The Case Manager will lead the process for the discharge needs of rehabilitation patients with emphasis on culture, financial considerations and family dynamics.

Responsible for assessments, coordination of discharge planning activities and continued reviews, this is accomplished by conferring with all parties involved and identifying appropriate options. Participate in the collaboration process which assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet patient health needs during their stay at an HCA facility.

This is a full time role working 37.5hrs per week

The attractive and competitive salary for this role is upto £45,000 and will be dependent upon skills and experience.

Duties & Responsibilities

  • Perform utilization reviews for all managed Care patients as appropriate
  • Resource/liaise between patient, hospital, physicians and third party payers to optimise patient’s treatment
  • Promote effective resource utilization and appropriate levels of care
  • Participate in multi-disciplinary team meetings to coordinate patient’s care plan and full discharge planning
  • Demonstrate behaviour supportive of team achieving department goals
  • Track and trend denial and appeals to identify issues and develop action plans for resolution
  • Act in accordance with the UK Data Protection Act to protect patient privacy
  • Support Case Manager team, serving as back up resource as required

Skills & Experience

As a RGN (registered general nurse) with substantial post graduate experience, you will ideally have experience of utilisation review.

Experience of managing multiple stakeholders across a multitude of seniority, you will also possess excellent communication skills and have a dynamic approach to problem solving.

Experience in a neuro-rehab environment is preferred.
