Specialist Nurses - Long Term Conditions Management - up to 45 hours per week.

Come and Join our caring team of GPs, Nurse Practitioners, GPs with Special Interests, Practice Nurses, and Healthcare Support Workers, supported by excellent Pharmacy, Dispensing, and Administrative teams.

We have vacancies for highly motivated and experienced Specialist Nurses: up to 45 hours per week, to deliver our care across both sites. Salary range is equivalent to band 7 and dependent on skills and experience. Ideally we are looking for someone to work full-time (37 Hours) and we can consider part-time positions dependent on skills and experience.

Preferably you will have experience within a primary care setting, have excellent clinical skills and be competent in pro-active management of patients with chronic diseases including Diabetes, Asthma, COPD, Hypertension and Cardiovascular disease.
