Job Summary

We are currently sourcing for a Senior Triage Nurse to join our Peninsula NHS Treatment Centre. The Outpatient Department is one of the more challenging but most rewarding places to work. The post holder will effectively manage the Triaging of UHP and choose and book referrals to Peninsula NHS Treatment Centres as per the OPCS codes and agreed referral criteria and guidelines. Effectively manage investigations carried out pre and post operatively that may impact on the patient pathway. Care UK Treatment Centres are patient focused and work closely with local CCGs and NHS Trusts to reduce waiting times. Several of our Treatment Centres are rated Outstanding by CQC. Key Responsibilities

• Provide effective referral triage. • Manage the awaiting referral triage work list and the second opinion work list within the agreed key performance indicators (KPI’s). • Ensure that results of diagnostic investigations previously performed are requested and available for outpatient appointments. • Accept or reject referrals as per CARE UK referral criteria. • Ensure that no inappropriate referral is accepted or rejected under the supervision of the senior support and Triage Nurse. • Train members of the Multi-disciplinary team to ensure they are competent and up-to-date in the referral triage procedures and protocols. • Communicate effectively with the clinical multi-disciplinary team and medical consultants. • Liaise and communicate effectively with Referrers, Sponsors, Radiology departments, Patient Booking Team, Consultant Anaesthetists and Surgeons, OPD and other external stakeholders. • Edit reasons for rejection on patient administration system as requested. • Ensure all tests carried out Pre and Post operatively are referred to the appropriate member of the multi-disciplinary for checking, sign off and scanning into the patients ICP. • Ensure all abnormal blood results are actioned appropriately in a timely fashion and letters are prepared and sent to the patient and their GP. • Ensure Maxims is updated accordingly with patient information to effectively manage the 18 week referral to treatment date. • Ensure no patient breaches and manage patients that are reaching their breach dates.

• Extensive surgical or medical care experience within an acute environment.
• Previous experience in assessment or triage of patients (either face to face or telephonic)
