Salary £33,250 to £43,050 per annum

Brilliant outcomes through integrated health, housing and care. That’s when it hits home.

This is your chance to be part of something brilliant at Home Group, where clinical interventions work hand in hand with daily support. You will be joining us on our exciting journey to become market leaders in New Models of Care by 2021. We are trailblazing the way in delivering brilliant long term outcomes for vulnerable people through integrating health, housing and care.

We have a brilliant opportunity for you to join our services across County Durham and Teesside, which provide support to adults (aged 18-65) who have mental health issues and multiple complex needs.  We support our customers to move on from 24 hour supported living, to semi-independent living, to living independently in the community – how amazing is that!

As a Registered Mental Health Nurse, you will be developing our Support Coordinators to ensure that high quality individualised care is delivered, which ensures safety, recovery, wellbeing and maximises customers independence.

As an experienced clinician, you will lead practice development, developing and delivering specialist training packages to our colleagues, embedding core competencies as well as oversight of clinical supervision.  You will also be the point of contact for individual support and coaching of the Manager of the services.  You will be really enhancing colleagues’ knowledge and skills, promoting regular, honest and supportive feedback.

What is even more brilliant, you will be able to really make your mark (there is no red tape or mounds of paperwork here!).  You will be shaping our services, developing essential standards and quality outcomes.  With customer care is at the forefront of everything we do, you will inspire others and advocate a philosophy of care that is person centred, maintains dignity, independence and ensures it is right for that person at that time. You’ll enable us to maintain high standards of support and intervention to contribute to potentially achieving the Care Quality Commission Essential Standards.

On a day to day basis, you will report to Dom, our Senior Client Services Manager with your clinical supervision provided by our Clinical Practice team.  The team love to catch up each week to discuss new interventions that have really helped our customers. They also make sure there is plenty of cake on the go (keep us a slice) and time in the agenda to discuss the latest episode of “I’m a Celebrity.” So who will be the queen/king this year?
