The Specialist Children’s and Young People’s Services (SCYPS) at East London Foundation NHS Trust are wonderfully pleased to announce, that in conjunction with Roald Dahl’s Marvellous Children’s Charity we are seeking to recruit a marvellously motivated and enthusiastic Community Paediatric Nurse to develop this innovative role as a Transition Care Community Nurse Specialist for Young People with special educational needs and complex disabilities in Newham, East London.

Each year SCYPS accepts approximately 350 young people aged 11-18 onto its specialist services caseload. Approximately 30% of teenagers on our caseload have a chronic health condition, and many young people accessing our services have complex needs. 

The successful post holder will have a key role in empowering young people with complex disabilities and special educational needs and their families to transition effectively into adulthood and to provide support, information and practical care during the period of navigating the move from Children's to Adult Community Services. You will work with front line staff in SCYPS and related teams in the community and acute services to strengthen the care we provide for young people with SEND and to improve their experience of transitioning to adulthood.

In partnership with Roald Dahl’s Marvellous Children’s Charity, we are looking for a highly motivated and dynamic community nurse with experience caring for young people with complex disabilities and special educational needs. We are looking for an exceptional nurse who would "never do anything by halves" and would be outrageously passionate about making a splendiferous difference in the lives of the young people and their families in Newham.
