We are looking for an experienced Practice Nurse to provide high quality, compassionate care to our patient population.  We are a large practice delivering care to a total of around 26,000 patients across four sites with a well-established nursing team of five Practice Nurses and five Health Care Assistants.

An exciting opportunity has arisen to join this team as the nurse manager, taking a leading role in developing the nurse and HCA team as well as providing a first point of contact for any arising nursing queries from within the team or the wider practice team.  The role would be suited to a Practice Nurse with significant experience in General Practice and looking to develop their career and take on a new challenge.  Support can be given in the development of the management aspects of the role, but an ability to provide clinical leadership to the team and drive the delivery of high quality care are essential.

As well as undertaking Practice Nursing as a part of the team, the successful candidate will need to have an interest in an area of Chronic Disease Management.

JOB DESCRIPTION

Job Title:                             Practice Nurse – Nurse Manager  

Reports To:                         Practice Manager (Admin), Practice Partners (Clinical)

Hours:                                  37.5 hours per week, part time considered minimum 30hrs per week

Location:                             Across all the Churchill Medical Centre & The Orchard Practice

PURPOSE

  • To work as a Practice Nurse Manager with responsibility for the management and coordination of the Nursing team, including Health Care Assistants
  • To provide high quality, compassionate nursing care to our patient population
  • To work within the multidisciplinary team of GPs, nurses and allied health professionals to deliver preventative medicine and chronic disease management to our patients
  • To provide nursing input in to practice policy development and practice decisions affecting the care of patients
  • To contribute to QOF, practice research, audits and other local or national directives
  • Provide a leading role in Infection Prevention and Control alongside the GP and administrative IPC leads

 

SPECIFIC JOB RESPONSIBILITIES

  • Provide high quality, compassionate nursing care to our patient populations alongside the nursing and healthcare assistant team
  • Undertake Practice Nursing duties including, but not limited to;
    • Cervical Screening
    • Childhood immunisations
    • Ear Syringing
    • Wound Care
    • ECGs
    • Audiology
    • Venepuncture
    • Hypertension management
    • Travel vaccinations
  • Take a lead in an area of Chronic Disease Management, such as Asthma, COPD or Diabetes
  • Work alongside GP leads to deliver practice targets such as immunisation uptake
  • To assess, diagnose and prescribe appropriately within your capability and nursing grade. Ensuring practice prescribing policy is adhered to
  • Participate in practice audit programmes for health promotion, target maintenance and patient care
  • Lead on Infection Prevention and Control in the practice
  • Work alongside the practice management team to provide a nursing perspective to developing practice policies
  • Coordinate and chair nurse meetings involving all members of the integrated nursing team on a bimonthly basis to disseminate internal practice and external information
  • Responsible for developing the role of the Health Care Assistant
  • Responsible for staff appraisals and development with the nursing team
  • To update and maintain accurate patient medication records on the practice clinical computer system, including advice given and action taken
  • Recognise and work within own competence and professional code of conduct as regulated by the NMC
  • Provide a good learning environment for patients, Practice Nurses and  Health Care Assistants
  • Be responsible for own continuing professional development
  • Support and encourage further education of nurses and HCAs within the practice team
  • Work collaboratively with the practice team
  • Participate in practice meetings, patient participation groups and other meetings to improve engagement with the role of the nursing team

Health & Safety

The post-holder will implement and lead on the full range of promotion and management of their own and others’ health, safety and security as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include (but will not be limited to):

  • Maintain an up to date knowledge of health and safety and infection control statutory and best practice guidelines and ensure implementation across the business
  • Using personal security systems within the workplace according to practice guidelines
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across the business
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards, and initiation of remedial / corrective action where needed
  • Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised
  • Keeping own work areas and general areas generally clean, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector managers

Qualifications

  • GSCEs (A-C) in Maths, English & Science or equivalent.
  • NMC approved Degree or Diploma of Higher Education in Nursing.
  • Registered Nurse.
  • Independent Nurse Prescriber- V300.
  • Supervisory Management qualification.
  • Long Term Conditions Management – Diabetes

Experience

  • Minimum of 5 years post-qualification experience.
  • Experience of working within General Practice.
  • Practical experience of computerised recording systems.
  • Experience of using own initiative.
  • Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on going competence in all aspects of the role.
  • Area of chronic disease management expertise – preferably diabetes.
  • Practical experience of managing and developing a nursing team.
