Due to the promotion of the current postholder, Torfaen Community Resource Team is seeking to recruit a Team Manager.

The CRT Team operates between 8am and 8pm from Cedar Unit in County Hospital and provides a high standard of nursing care and therapy led rehabilitation to patients and clients in their own homes, residential/nursing homes and other care settings in Torfaen. The service also provides intermediate care to patients to allow residents to remain at home and receive treatment as close to home as possible in line with the Health Boards Place Based Care Strategy.

The postholder will act as the senior operational and professional lead of the service and be responsible for the service delivery in line with the strategic vision of the Health Board and Local Authority, working in partnership with colleagues in ABUHB and TCBC.

The post holder will  lead developments within the Borough, clarifying and coordinating roles and responsibilities of staff, ensuring appropriate lines of communication and forums are in place to provide effective and professional decision making. They will also collaborate with Professional Heads for Therapy, Nursing and Social Care and other services across the Health Board and Local Authority , to ensure that strategic development plans meet the developing needs of the local population . The post holder will manage the budget for the team and will assist the Borough to form and implement the future strategic direction for CRT and other related services. They will also work closely with the Locality Planning and Intermediate Care Leads in both Health and Social Care to ensure a strategic fit.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
