Critical Care Outreach Nurse

Job Description 

We are seeking a motivated and experienced critical care nurse to join our dynamic Critical Care Outreach team. We are looking for someone who demonstrates sound clinical knowledge, with relevant experience in an acute/critical care setting to support the delivery of a high quality nurse led Outreach Service 24 hours a day, 7 days a week.

The CCOT is a multidisciplinary team of experienced critical care practitioners that assist with the nursing care of patients who are acutely unwell, deteriorating or at a high risk of deterioration. This is achieved by supporting education and working with the ward staff to optimise the quality, care and experience of our patients.

The CCOT are also members of the hospital resuscitation team providing clinical response to peri/cardiac arrest and major haemorrhage events. The Team also supports the safe transfer of patients from low acuity wards to HDU or ITU. 

The CCOT at WH plays an active role in ensuring the standards of care across the hospital are maintained. They participate in implementation of action plans to improve safety and care standards. The team also works alongside the Site team on our hospital at night initiative.

As a member of the CCOT team you will be providing education and clinical support to the staff in the clinical areas. The post holder will deliver evidence based care and demonstrate an enthusiasm for the specialty and a willingness to accept and initiate change. 

Skills and Experience 

• NMC Registered Nurse
• 3 years critical/acute care experience.
• ALS provider or willingness to undertake course
• Extensive experience within  the critical care environment
• Significant critical care experience at shift leader level.
• Postgraduate qualification in critical care / acute care.
• Evidence of Continuing Professional Development.
• Leadership training (desirable).
• Significant experience of mentoring/supervising staff with commitment to the professional development of others.
• Experience in audit/service improvement project(s)/change introduction and management.
• Motivated and forward thinking individual with excellent communication skills.
• Good time management and prioritisation skills.
• Awareness and understanding of current health issues and initiatives, utilising research in clinical practice.
• Able to use and embed evidence based practice within teams.
• Experience in advanced respiratory ventilation, cardiovascular and renal care.
• Computer literacy.
• Problem solving – responds to unexpected issues and provides first line interventions to manage the situation.
• Honesty and Integrity.
• Highly self-motivated and driven to succeed and make a difference – possess high work standards, desire to do things better and to be more effective and efficient, measures progress against targets and implements change accordingly.
• Able to adapt to ensure achievement of objectives within constantly changing situations and environments. Supports others to achieve the same goals. Demonstrates tenacity and ability to cope in high pressure situations.
• Physically able to perform all aspects of clinical nursing role to include manual handling of patients.

This is a full-time role working 37.5hrs per week - flexible shifts on internal rotation, including long days, nights and weekends.
