Are you passionate about palliative care? Hours:
Full-time - 37.5 hours over 7 days (Including some Saturdays or Sundays on a rota)
Part-time will also be considered.
What is the role?
As a Community Clinical Nurse Specialist (CNS) with us you would be responsible for supporting and working alongside primary care networks in delivering high quality end of life care. You will work alongside GPs, community nursing teams and wider health care professionals to ensure a flexible, responsive and patient-centred approach to care in the community setting. You will make clinical decisions including assessing the physical, psychosocial and spiritual needs of patients and their families.
This post involves working within a multi-disciplinary team offering specialist palliative care to patients in their own homes as well as virtually and over the telephone. You will also promote and develop the specialist advisory service and supportive role of the CNS and Palliative Care team. There will be responsibility for influencing the provision of high quality specialist nursing care and delivering both informal and formal education programmes.

About you:
- Registered Nurse
- Minimum of five years post registration experience
- Experience and knowledge of palliative care
- Understanding and experience of multi-disciplinary team working
- Experience of using reflective processes as a framework to improve practice
- First level degree, or willingness to undertake in palliative care or a related area
- Full UK driving licence holder and access to a car for work
It would also be beneficial if you have:
- Clinical Nurse Specialist experience
- Advanced clinical decision making skills
- Advanced communication skills
- Informal and formal teaching experience
- Resilience and effective strategies and approaches to coping with the emotional demands of the role

Prospect Hospice is an independent hospice supporting a community of over 300,000 people in Swindon, Marlborough and north east Wiltshire as well as the villages of Lechlade and Fairford in Gloucestershire. We are the only dedicated provider of specialist end of life care services in the area. The hospice’s wide variety of professionals support people in the community and in care homes. Care is also extended to the families and friends of patients that we care for.
