In our exciting new role of Regional Transforming Care Manager, you’ll lead the safe, efficient and effective care for up to 60 complex customers across your region, managing our Clinical Registered Managers, Deputy Managers and PBS Regional Lead (working closely with our clinicians and front line care and support teams too). This is a brilliant opportunity to shape this role and really make it your own, driving service excellence and brilliant outcomes for our customers.  

You’ll lead our teams to support our customers who have complex support needs, including learning disabilities, autism, a diagnosis of mental health conditions, personality disorders, communication differences, alongside ongoing physical health conditions. They may also have a forensic background and support needs related to their behaviour.   

As our designated person for safeguarding, our plan is for you to hold and maintain regional registration for our services under TDDI (Treatment of Disease, Disorder and Injury), ensuring that our support practices and interventions are delivered in line with our Living Independently Feeling Enabled (LIFE) support practice model (we’ll tell you more about this when we meet you).  Through well-led clinical assessments and transition planning, our customers will be able to leave long-term institutions, overcome barriers, live safe and high-quality lives. How amazing is that?  

Responsible for CQC regulation and registration, you’ll implement effective systems and processes to manage quality of care, as well as ensuring that all of our services maintain highest levels of safety which support customers to integrate back into a community environment.

Where you’ll work

You’ll be based from one of our regional South offices and can also work from home. You’ll however be covering a large geographical patch currently including Kent, Swindon, Birmingham and Hertfordshire with more locations over the next two years. You therefore, must be willing to travel and work in our different services/locations, have a driving licence and access to a vehicle for business purposes.

A bit about you

You’ll have substantial senior manager experience delivering brilliant outcomes for customers who have complex support needs and presentations in a health or social care setting. Experienced in managing large budgets, overseeing and ensuring the financial performance of operational services, you’ll be able to manage teams based across multiple locations. 

You’ll ideally have a degree and/or clinical qualification or equivalent specialist training with substantial CQC Registered Manager experience is brilliant by us too! You’ll really impress us with your ability to lead your team and services to exceed quality standards and requirements, as well as your passion to unleash the potential of others, making it a great place to work for our colleagues.

Here at Home Group we have invested heavily in our future and use current and up to date equipment. You’ll be issued with the latest kit, which could include a Surface Pro and mobile. So, it's important to us that you are technologically savvy (or at the very least not a technophobe!)

As you’ll be covering a large geographical patch, you need to be able to drive, as well as having access to a car insured for business purposes. Don’t worry, we’ll pay you expenses to cover this use!

An Enhanced Disclosure Check will be required for this role. The great news is that we'll pay for this to be done if you don't have a transferable one.
