About Us:

Coloplast Wound Care is developing a new service model within a Primary Care setting. We are looking to recruit a highly skilled Tissue Viability Nurse to work within a transformational project. The successful role holder will be part of a team creating a nursing service dedicated to delivering excellence in clinical care, consultancy, education and audit.

In this role you will build relationships with both local Primary Care Networks, GP Federations and Coloplast. You will undertake audits, deliver education and will coach other health care professionals along with carrying a clinical caseload.

This role will report into a Clinical Nurse Manager but will also have a dotted line report, from a governance perspective, into the Primary Care area he/she is working in.

This role supports the Hartlepool & Stockton on Tees area, and anyone applying must live in a reasonable travel distance to the area.

The essential duties of the role:

  • Setting up a tissue viability service in an agreed, specified area where a partnership opportunity has been identified and agreed upon by both Coloplast and local stakeholders.
  • Lead the local implementation of the new Tissue Viability service model
  • Plan, implement and run the local project, engaging all relevant stakeholders
  • Drive and influence changes required for successful outcome of the project, for Coloplast, Primary Care Networks, GP Federations and Patients
  • Plan service delivery aligned to local needs.
  • Use a range of skills to proactively monitor the overall progress, resolving issues and initiating corrective action as appropriate
  • Demonstrate leadership capabilities to influence, educate and train local health care professionals on the highest levels of tissue viability practice
  • Undertake the delivery of clinical caseload using an evidence-based approach to promote excellence
  • Acting as a Clinical expert to a range of health care professionals within Primary Care
  • Teaching patients/carers and providing support to patients/carers to ensure self-care by patients and/or their carers is safe and effective

To be successful in this role you will have the following education and skills:

  • Degree level Registered General Nurse.
  • Post graduate qualification in Tissue Viability.
  • Experience at a senior nursing level of leading and driving a service of change and ability to evidence the outcomes of that change.
  • Recent clinical expertise and clinical competence in tissue viability including leg ulcer management.
  • Demonstrable competence in leading teaching programmes and in presentation skills.
  • Experience in undertaking/collaborating in research, clinical audit and service improvement in order to understand and source new solutions.

The successful candidate will be able to display empathy and have an excellent ability to develop trust.  To do this you will have excellent communication skills, including listening, interpersonal and influencing skills.

The salary banding for this role aligns with an NHS Band 7 / 8a.
