An opportunity to develop the support and expertise that KEMP provides to the care homes within the Wyre Forest. This role will ensure the providers of care for people in care homes have access to support and training, ensuring people can remain and die in their preferred place of care, supported by trained staff trained to manage their needs.

An opportunity to utilise leadership skills whilst ensuring high quality clinical skills are maintained. The role will influence the development and delivery of expert care within Care Homes and KEMP Hospice. The post holder will form part of the KEMP Hospice MDT, but will work across a range of services, with broader organisation-wide focus on extending services for patients currently living in or discharged into 24-hour care facilities.

This role will be instrumental in promoting and supporting the development and delivery of advance care planning and end of life care within the care homes in the Wyre Forest.

Main duties of the job The post holder will work closely with all the 44 Adult Care Homes within the Wyre Forest to identify the needs of the resident and plan care using best evidence-based practice. Other key relationships will be Wyre Forest Health Partnership, Wyre Forest Network of independent practices, Community Specialist Palliative Care Team, Herefordshire & Worcestershire CCG, Herefordshire & Worcestershire ReSPECT Project Team, and other Hospice and Third Sector Services / Teams.

The post holder will support the KEMP Hospice wider team to improve the uptake of Advanced Care Planning and ReSPECT discussions for people living in their own homes and those living with Frailty in the last years of life. As the role develops there is the potential to hold a Nurse Led Clinic at the KEMP Hospice supported by the KEMP MDT.

Job description Job responsibilities The post holder will be expected to-

Support:

  • Support the care homes within the Wyre Forest to ensure all residents and/or their loved ones have an opportunity to discuss and complete Advance Care Plans, ensuring their wishes and preferences are recorded.
  • Provide support and advice to care home staff when residents are nearing the end of their life.

Educate:

  • Provide the care home staff, both registered nurses and care support workers with relevant education to enhance the end-of-life care provision within the care homes.
  • Use opportunities such as care home open days and dying matters awareness week to raise residents and the publics awareness on Advance Care Planning (ACP).
  • Ensure that residents and/or their loved ones are educated to make informed decisions.

Improve:

  • Support care home managers to develop strategies to identify and support service improvements, audits for raising standards in advanced care planning and end of life care.
  • Working closely with PCNs and local MDTs, gather feedback and use it to reflect and review support, care and the education that is provided. Regular Auditing and case reviews on unplanned hospital admissions and incidents.
  • Provide information and data for quarterly reports to look at activity levels and outcome measures.
  • To formulate, implement, disseminate and monitor amended and new policies and procedures, ensuring agreed standards are maintained, having a thorough working knowledge of policies and procedures, and identifying where existing standards should be reviewed, in a way that ensures commitment, understanding, compliance and best practice.

Integrate:

  • Build good working relationships with the care home staff and other healthcare professionals such as GPs, ANPs and SNPs.
  • Be a visible asset for supporting ACP and improving end of life care within the care homes of the Wyre Forest.
  • Liaise with appropriate external colleagues to ensure seamless care and encourage their involvement with improving end of life care and ACP within care homes.

Communicate:

  • Be an exemplar of advanced communication skills utilising recognised models of communication with residents, families, and healthcare professionals.

Person Specification Experience Essential

  • Minimum of 5 years post qualification experience Experience of working in either community, specialist palliative care, frailty, or general palliative care services Experience of productive multi and inter-disciplinary working Experience of Teaching & Assessing

Desirable

  • Experience of working in a care home environment Experience of implementing change in a service environment

Skills and knowledge Essential

  • Excellent communication skills and interpersonal skills Ability to analyse complex facts and situations and develop a range of options Ability to think laterally, creatively, and innovatively Ability to make decisions autonomously, when required, on difficult issues, working to tight timescales

Desirable

  • Ability to critically analyse research and evidence-based practice Experience of Audit Experience of identifying and interpreting National Policy

Qualifications Essential

  • First Level Registered Nurse Evidence of a post qualifying Degree or Diploma / Recognized qualification in either Specialist Palliative Care, Frailty, or related subject Evidence of ongoing CPD Evidence of Advanced Communication Skills Training and / qualification

Desirable

  • Non-Medical Prescriber Advanced Physical examination Qualification or equivalent level of experience
