An exciting opportunity has arisen for an Advanced Clinical Nurse Practitioner Band 8a to lead a brand new Intermediate Care Unit. Heathlands is an exciting new venture in conjunction with Bracknell Forest Council to provide both Immediate and Dementia Care to Bracknell and the surrounding area. Frimley Health are pleased to be the official provider of the Immediate Care Unit service, which is due to open in January 2022.
The Immediate Care Unit is a 20 bedded unit providing support to patients transitioning between acute hospital environment and the patient’s home.
We are at the forefront of improving upon the existing processes involved in patient care, management and flow in coordination with our different Community and Acute hospital teams to deliver better experiences and outcomes for our patients.
We are looking for a dynamic and motivated individual who is passionate about caring for our vulnerable and complex patient group and will provide 5 star patient care and experience in line with our Trust values.
In addition to providing:
Expert Practice: To deliver high quality care to patients, enabling other practitioners to maintain professional expertise, and exercising a high degree of personal autonomy within their scope of professional practice.
Education, Training & Development: To act as a constant source of clinical and theoretical knowledge for all grades and disciplines of staff as well as patients and their significant others, providing support and clinical advice in specialist areas, based on evidenced based research
Professional Leadership & Consultancy: To exhibit leadership skills to influence, support and motivate others to continuously improve quality of care and standards of practice.
Practice, Service Development, Research & Evaluation: To contribute to the development of professional practice, through the promotion of evidence-based practice, audit of standards of care, research and evaluation of practice within specialist fields.
At Frimley Health Foundation Trust you will be supported from the moment you start in the Trust with a comprehensive Trust induction, for our Newly Qualified Practitioners we offer a 12 month bespoke preceptorship programme with 14 days protected study days designed to teach and develop our practitioners to the very highest standard.
For experienced professionals we offer a range of mentorship and management essential training alongside our clinical training programmes.
