The CCN service currently has a vacancy for a Band 6 member of staff. This is an exciting opportunity for a motivated individual at time of significant change. Training, education and mentoring support will be given. 

 The post holder would be involved in:  

  • Close liaison work with the acute paediatric team / CEAU to support early discharge home, prevention of admission and service development within the context of Care Closer to Home.
  • Participate in CCTH audit.  
  • Continue to develop and enhance the clinic services offered within CCTH and the wider CCN team.  
  • Clinical support within the special needs schools. 
  • Operationally manage, provide leadership, professional advice and care for the Children within CCTH Service and support the wider CCNS teams. 
  • Work in partnership with a variety of multi-disciplinary agencies to provide skilled nursing care to children and young people in a variety of community settings. 
  • Participate in meeting the developing needs of CCTH
  • Training and education of staff at all levels 
  • Support and assess the HCSW with delegated responsibilities 

Substantial paediatric experience is required, and previous experience working with children with acute and complex healthcare needs, mentorship qualification is preferred. 

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
