• NMC PIN – RMN Qualified
• Community experience preferred but not essential

Responsible for, but not limited to the following:

• Visiting patients in their own homes to deliver nursing care
• Overall management of caseload of patients
• Developing and following care plans
• Liaising with fellow professionals to ensure all patients needs are met.
