We are seeking 2 experienced, enthusiastic, motivated and committed Registered Mental Health Nurse, working separately as follows:

  • Based in the Lincolnshire to cover Hull, Lincoln, Gainsborough and Sutton-on-Sea.
  • Based in Staffordshire to cover Sandbach Chester, Leek, Birmingham.

A fast-growing company, Northern Healthcare provides mental health recovery services across many regions within the UK. Our services consist of supported living and community outreach services which provide our residents with access to 24-hour support from experienced clinical and support staff.  

Main duties of the job

In line with the philosophy of each service, the post holder will be responsible for the assessment, planning, implementation, and evaluation of care for the identified residents utilising a case management approach.  

Your main focus in this role is to deliver specific interventions on an individual basis, providing effective evidence-based care to residents  

Communication and liaison with several disciplines are essential to promote effective delivery of the clinical care pathway to meet residents’ needs.

You'll provide support and direction to team members and assist in the smooth coordination and running of the service.

Where you’ll be working and hours of work

  • Based in Lincolnshire to cover Hull, Lincoln, Gainsborough and Sutton-on-Sea.
  • Based in Staffordshire to cover Sandbach Chester, Leek, Birmingham.

The role are offered on a full-time basis – and can support flexible working arrangements appropriate to service needs.  The full-time position is 37.5 hours per week.

Key responsibilities

The post holder will deliver specific interventions on an individual basis, providing effective evidence-based care to;

  • Develop transition, admission, and discharge plans, follow up referrals and assessment for suitability for the services.
  • Develop resident centred support plans and robust risk management and crisis plans in collaboration with the resident, carer, and other professionals involved. Review, evaluate and replan support plans.
  • Ability to offer comprehensive mental health assessments underpinned by evidenced-based knowledge and collates information with a view to have a comprehensive case
  • Ability to formulate support plans, risk management plans formulate risk and be part of a wider team to assess risk.
  • Out of hours on-call for crisis management on a rotational basis.
  • Referral and liaison to secondary
  • Ensure timely and accurate documentation and maintain confidentiality within legal, organisational and ethical
  • Ability to manage high risk and complex presentations in a community with positive therapeutic risk
  • Ability to undertake medication management, and medication audits and complete medication competencies to care staff.
  • Understanding of Mental Health Act (1983) and monitoring of required
  • Utilise strategies to enhance recovery for our residents.
  • Ability to advocate on behalf of residents to support them to access a range of services that, as far as possible, reflect their needs and wishes.
