Primary Care Practice Nurse

We are seeking to increase our Primary Care Practice Nurse provision.  Candidates will ideally have an interest in diabetes, but this is not essential.  We are looking for full or part time hours dependent on candidate response. We are a friendly, supportive, innovative, and progressive Practice.  We operate a ‘can do’ approach towards delivering high quality care to our patients. We recognise continuous professional development is important and offer support with this and revalidation.  You will be working alongside our Assistant Health Care Practitioner – with a team working focus. Some home visits maybe required.  We have a fantastic clinical and administrative team for our patient population and our list size is currently expanding from six thousand to twelve thousand enabling us to operate a larger nursing team.

Essential competencies

Must be RGN

Nursing degree or Registered Nurse degree apprenticeship

Chronic disease management (ideally with an interest in diabetes)

Phlebotomy and administering injections are essential

Flexible approach towards patient care and recall

A positive, proactive attitude and excellent communication skills are essential

Desirable competencies

Experience in a Primary care/ community setting

Diabetes interest

Experience of QOF and Standards

Working knowledge of EMIS Web, essential

Job Type: Part- or full-time hours: From 24 to 37 hours per week
