Are you ready to specialise? The Rennie Grove Hospice at Home teams provide palliative symptom control and end of life care in patients’ homes, which challenges you to be more flexible and responsive as a nurse, while delivering holistic palliative care to the patients and their families. 

As a Hospice at Home Nurse or Senior Hospice at Home Nurse with Rennie Grove in the Buckinghamshire Hospice at Home team (Ridgeway, South Bucks and Wycombe), you’ll have the opportunity to learn, develop and become an expert in your chosen field of nursing. 

Acting as a patient advocate - ensuring continuity of care

You’ll use your nursing skills and knowledge, complemented by the education, support and mentoring we’ll provide, to make sure all a patient’s end-of-life care needs are addressed. You’ll work with multiple agencies, advising, explaining and acting as a patient advocate in conversations with GPs, paramedics and other healthcare providers. This multi-disciplinary collaboration, alongside excellent communications and teamwork within the charity, means your patients will have continuity of care and be able to focus on quality time with their family.

Holistic nursing care

You’ll have the time, training and support to gain a more in-depth understanding of patients’ needs and how to address them. Your work isn’t task-orientated, but fully focused on the care and support each family needs for their love don to spend their final days at home.

Visiting patients in their own homes, you’ll be providing specialist palliative care alongside emotional and psychological support for the whole family, building a rapport over weeks, months or even years so that symptoms are controlled, hospital admissions prevented and families able to spend quality time together, at home.

Responsive and anticipatory care

Specialising in palliative care, especially in a community setting, requires you to be confident in different styles of nursing. You’ll need to anticipate likely developments, listening carefully and trying to predict and pre-empt problems and prevent emergencies. But you’ll also need to act decisively in rapid response to a patient’s sudden deterioration. Although you’re always anticipating and planning for the next stage in a patient’s illness, you need the confidence to put that plan into practice at short notice. Rennie Grove’s induction, education and CPD programme can build on competencies and give you that confidence.

Experience

Experience of palliative, cancer or community care is desirable but not essential. As long as you have general or specialist post-registration nursing experience as a Band 5 nurse, are a good sensitive listener and team player with the confidence and decision-making skills to assess and respond rapidly to a patient’s evolving needs, then this could be the role for you.

This role will be a Rennie Grove Band 5 Hospice at Home Nurse or a Rennie Grove Band 6 Senior Hospice at Home Nurse, depending on experience.

You’ll need:

  • first level registered General Nurse desirably with a module in palliative care or cancer care
  • current post registration experience at Band 5 or above carrying out general or specialist nursing care
  • proven experience of effective team working
  • excellent judgement and good decision making skills
  • ability to assess, plan, deliver and evaluate nursing care
  • caring and empathetic approach
  • commitment to Rennie Grove purpose
  • your own car and a driving licence.
